We have provided the initial patient registration intake package on-line for your convenience. Please print, complete, and bring all completed forms with you to your first visit. Or you may scan and email the completed forms to Patient_Registration@TheTherapySpot.com This will expedite the registration process for you. All forms are in Adobe® Acrobat® PDF format for your convenience.
We accept most Medicaid and private insurances. If you are unsure, you can obtain a pre-approval from your insurer before your first visit.
